Summary

Cognitive decline is more frequent in centenarians than the general elderly population. Good cognitive function and health are essential, but not sufficient, for maintaining autonomy in very old age.

Background:

  • While numerous studies document age-related cognitive deterioration, few explore its impact on the clinical status and autonomy of long-living individuals.
  • Understanding cognitive function in centenarians is crucial for assessing their overall health and independence.

Purpose of the Study:

  • To investigate cerebral deterioration in centenarians.
  • To correlate cognitive function with clinical conditions and autonomy in centenarians.
  • To determine the prevalence and impact of cognitive disorders in extreme longevity.

Main Methods:

  • Analysis of 346 centenarians (age 100-109) from the Italian multi-center study on centenarians (IMUSCE).
  • Assessment of cognitive function using the Mini-Mental State Examination (MMSE).
  • Evaluation of functional autonomy via Instrumental Activities of Daily Living (IADL) and clinical conditions (Groups A, B, C).

Main Results:

  • 54.1% of examined centenarians had a normal MMSE score.
  • Cognitive disorders were significantly more frequent in centenarians (13.1%) compared to the general elderly (5.1%).
  • Severe cognitive impairment precluded total autonomy; only 1.7% of centenarians were fully independent, possessing good cognition and clinical health.

Conclusions:

  • Cognitive disorders are highly prevalent in centenarians.
  • Normal cognitive scores and good clinical conditions are necessary but insufficient for complete autonomy in Instrumental Activities of Daily Living.
  • Maintaining autonomy in extreme longevity requires a complex interplay of cognitive, clinical, and functional factors.
