Summary
This case report details a rare maxillary sinus mucocoele in a young male, successfully treated with radical surgery. The findings highlight the importance of surgical intervention for infected sinus mucocoeles impacting local structures.

Findings:
- Surgical extirpation via Caldwell-Luc procedure revealed a mucocoele with squamous metaplasia and submucosal inflammation.
- Histopathology confirmed the mucocoele diagnosis.
- Post-operative recovery was uneventful with no recurrence at six months.
Implications:
- Rare maxillary sinus mucocoele presentation requires prompt surgical management.
- Radical surgical operation is indicated for infected mucocoeles causing local anatomical compromise.
- The width of the maxillary ostia may contribute to the rarity of these lesions in this location.
