Minute scirrhous hepatocellular carcinomas undergoing different carcinogenetic processes

Scirrhous hepatocellular carcinoma (HCC), a rare liver cancer subtype, can arise from dysplastic nodules or develop de novo in cirrhotic livers. Further research is needed to clarify its early carcinogenic pathways.

Background:
- Scirrhous hepatocellular carcinoma (HCC) is a rare subtype defined by significant fibrosis.
- Early carcinogenic processes and pathological features of scirrhous HCC are not well understood.
Observation:
- Two cases of minute scirrhous HCC with distinct origins were analyzed.
- Case 1: A 54-year-old man with HCV-related cirrhosis had a tumor with a nodule-in-nodule appearance, featuring scirrhous HCC within a high-grade dysplastic nodule.
- Case 2: A 75-year-old woman with NASH-related cirrhosis had an incidentally found scirrhous HCC developing de novo within the cirrhotic liver, without dysplastic nodule components.
Findings:
- Scirrhous HCC can originate from pre-existing dysplastic nodules.
- Scirrhous HCC can also arise independently (de novo) in the setting of liver cirrhosis.
Implications:
- These findings suggest divergent pathways for scirrhous HCC development.
- Understanding these distinct origins is crucial for early diagnosis and targeted therapies for scirrhous HCC.
