  • Hepatocellular carcinoma (HCC), predominantly hepatitis B virus-related, had the poorest survival.
  • Most primary malignant tumors presented at disseminated stages.
  • Elevated alpha-fetoprotein was common in HCC and hepatoblastoma; liver function tests were often normal.
  • Metastatic tumors showed distinct imaging features (hypo-echogenicity, less vascularity) compared to primary tumors on ultrasound and CT.

Conclusions:

  • Early diagnosis of primary malignant liver tumors is critical, ideally before symptom onset.
  • Children with chronic hepatitis B require regular screening (alpha-fetoprotein, ultrasound) even with normal liver function tests.
  • Imaging modalities like ultrasound and CT can effectively differentiate between primary and metastatic pediatric liver tumors.
