Flow Sheet01:17

Flow Sheet

Flowsheets are valuable tools in nursing documentation. They enable healthcare professionals to efficiently record and monitor various patient assessments and measurements in a consolidated format.
Here's a closer look at the examples of flowsheets commonly used by nurses:

Methods of Documentation VI: Case Management Model

The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic illness...

Heart Failure VII: Nursing Interventions

The first step in nursing management of a patient with heart failure involves thoroughly assessing the patient's medical history.Subjective Data: Obtain the patient's medical history of coronary artery disease, hypertension, myocardial infarction, and symptoms like dyspnea, orthopnea, and paroxysmal nocturnal dyspnea. Objective Data: Conduct a physical examination to identify findings such as jugular vein distention, pulmonary crackles, tachycardia, murmurs, peripheral edema, and vital signs,...

Chronic Kidney Disease IV: Nursing Management

Nursing management is essential for preventing complications, maintaining stability, and improving patients' quality of life in chronic kidney disease (CKD). By using a structured approach, nurses help slow CKD progression and support effective patient care.1. Comprehensive patient assessmentEffective management begins with nurses reviewing the patient’s medical history, and identifying key risk factors like diabetes, hypertension, and nephrotoxic drug use. Nurses assess signs of fluid...

Hemodialysis III: Nursing Management

The nursing management of a patient undergoing hemodialysis includes several critical steps, starting with a thorough assessment before the procedure. Before the Hemodialysis ProcedureFirst, record the patient's vital signs—blood pressure, heart rate, respiratory rate, and temperature—to establish a baseline. This baseline is essential for detecting conditions such as hypotension that could impact the patient's response to dialysis. Document the patient's pre-dialysis weight, as this measurement...

Heart Failure VI: Adjunct Therapies

Additional therapies for treating patients with heart failure (HF) may include procedural interventions, supplemental oxygen, the management of sleep disorders, and nutritional therapy.Procedural InterventionsImplantable Cardioverter-Defibrillator: For patients at risk of life-threatening arrhythmias due to severe left ventricular dysfunction, an Implantable Cardioverter-Defibrillator (ICD) can detect and terminate these arrhythmias, preventing sudden cardiac death and improving survival rates.

Summary

A clinical pathway for elderly patients with fractured neck of femur significantly improved fluid management. This led to reduced perioperative hypotension and increased intravenous fluid administration.

Area of Science:

  • Geriatric Medicine
  • Surgical Patient Management
  • Clinical Pathway Development

Background:

  • The National Confidential Enquiry into Perioperative Death's (NCEPOD) 1999 report highlighted critical care gaps for elderly patients with fractured neck of femur (NOF).
  • Recommendations focused on improving management for patients admitted via Accident and Emergency (A&E) with NOF, particularly concerning fluid management.

Purpose of the Study:

  • To audit and improve fluid management in elderly patients with fractured NOF.
  • To assess the impact of a multidisciplinary clinical pathway on patient outcomes.

Main Methods:

  • An initial audit of fluid management in elderly patients with fractured NOF was conducted.
  • A multidisciplinary clinical pathway was developed and implemented.

  • The audit was repeated in 2002, 2003, and 2005 to evaluate the pathway's effectiveness.
  • Main Results:

    • Significant reductions in the incidence of perioperative hypotension were observed between 2000 and 2005.
    • A notable increase in the percentage of patients prescribed and receiving intravenous fluids was recorded (p<0.05).

    Conclusions:

    • A protocol-based clinical pathway, stemming from NCEPOD recommendations, substantially enhanced fluid management for patients with fractured NOF.
    • The implemented pathway demonstrated a positive impact on perioperative care for this patient group.
