Area of Science:

  • Vascular Inflammation
  • Rheumatology
  • Epidemiology

Background:

  • Giant cell arteritis (GCA) is a systemic vasculitis primarily affecting individuals of Northern European descent.
  • The incidence of GCA in Asian populations, particularly those of East Asian ancestry, is considered exceptionally low.

Observation:

  • This report presents two histologically confirmed cases of GCA in elderly Chinese men.
  • One patient exhibited classic GCA symptoms, while the other presented with subtle, occult disease manifestations.

Findings:

  • The cases challenge the notion of GCA being virtually unknown in Oriental populations.
  • The presentation of GCA can vary, including atypical or occult forms, even in ethnic groups where it is considered rare.

Implications:

  • These findings suggest that GCA may be underdiagnosed in East Asian populations due to atypical presentations or lower awareness.
  • Further epidemiological studies are warranted to determine the true incidence and clinical spectrum of GCA in diverse ethnic groups.
  • Clinicians should maintain a high index of suspicion for GCA in elderly patients of East Asian descent, regardless of typical symptomatology.
