Background:

  • Nocturnal voiding, or waking up to urinate at night, is a common issue affecting many adults.
  • The impact of nocturnal voiding on sleep quality and overall health-related quality of life (GHQL) requires further investigation, particularly distinguishing between bothersome and non-bothersome experiences.

Purpose of the Study:

  • To investigate the prevalence of bothersome and non-bothersome nocturnal voiding.
  • To compare the differences in lower urinary tract symptoms (LUTS), sleep quality, and GHQL between individuals experiencing bothersome and non-bothersome nocturnal voiding.

Main Methods:

  • A community-based postal questionnaire survey was conducted with 2,205 Japanese men and women aged 41-70 years.
  • Participants completed validated questionnaires including the International Prostate Symptom Score and Pittsburgh Sleep Quality Index (PSQI).
  • Subjects with one or two episodes of nocturnal voiding per night were categorized based on whether it caused sleep trouble, and their LUTS, sleep, and GHQL were compared.

Main Results:

  • Prevalence rates were: once-bothersome (14.2%), once-non-bothersome (31.4%), twice-bothersome (6.8%), and twice-non-bothersome (7.6%) nocturnal voiding per night.
  • The twice-bothersome group reported the worst LUTS, while the once-non-bothersome group reported the best.
  • Sleep quality and GHQL were significantly affected by sleep trouble due to nocturnal voiding, rather than the frequency itself. The twice-non-bothersome group often had better scores despite more frequent voiding.

Conclusions:

  • Sleep quality and general health-related quality of life are considerably impacted by sleep disturbances associated with mild (once or twice) nocturnal voiding.
  • The subjective experience of bother is a critical factor influencing the negative effects of nocturnal voiding on well-being.
  • Interventions addressing sleep quality may be crucial for managing the impact of nocturnal voiding.
