Aquagenic syringeal palmar keratoderma causes temporary, whitish, bubbly palms after water exposure. Topical aluminum chloride effectively treated this rare condition.

Background:

  • Aquagenic syringeal palmar keratoderma (ASPK) is a rare condition characterized by transient palm changes upon water contact.
  • This case highlights the clinical presentation and histopathological findings of ASPK in a young adult.

Observation:

  • A 21-year-old woman developed whitish, bubbly palms within minutes of water immersion.
  • The lesions presented as a sponge-like appearance with tiny white papules and resolved after drying.

Findings:

  • Histopathology revealed dilated intraepidermal eccrine ducts and a spongy corneal layer.
  • The clinical and histopathological findings confirmed the diagnosis of aquagenic syringeal palmar keratoderma.

Implications:

  • Topical aluminum chloride provided a rapid and effective treatment for ASPK.
  • The condition showed no recurrence after a 1-year follow-up, suggesting a good prognosis with appropriate management.
