Area of Science:

  • Gynecologic Oncology
  • Reproductive Medicine
  • Maternal-Fetal Medicine

Background:

  • Endodermal sinus tumor (EST) is a rare ovarian germ cell tumor with rapid growth.
  • Occurrence of EST during pregnancy is exceptionally uncommon.

Observation:

  • A 34-year-old pregnant patient was diagnosed with stage IC EST at 22 weeks gestation.
  • Treatment involved four cycles of cisplatin monotherapy followed by three cycles of combination chemotherapy (cisplatin, bleomycin, etoposide) during pregnancy.
  • The patient achieved complete remission 28 months post-treatment, maintaining good health.

Findings:

  • A healthy infant was delivered via cesarean section at 35 weeks gestation.
  • The infant exhibited normal development and examinations during the first two years of life.

  • Cisplatin monotherapy demonstrated effectiveness and safety during pregnancy.
  • Implications:

    • Adjuvant therapy significantly improves the prognosis for ovarian endodermal sinus tumors.
    • Cisplatin-based chemotherapy can be a viable and safe treatment option for EST during pregnancy.
    • This case highlights successful management of a rare gestational malignancy, ensuring positive maternal and fetal outcomes.
