Summary

Acute thalamic stroke can cause chronotaraxis, or time disorientation. This study links this specific symptom to thalamic lesions, particularly in the mediodorsal nucleus, and suggests good recovery potential.

Background:

  • Acute isolated disorientation of time, known as chronotaraxis, is a rare symptom of thalamic stroke.
  • Previous reports have not detailed acute thalamic chronotaraxis with associated Magnetic Resonance Imaging (MRI) findings.

Observation:

  • This study describes five patients presenting with chronotaraxis following a thalamic stroke.
  • The patients exhibited disorientation regarding the date, inability to ascertain the current time, and inaccurate estimation of elapsed time.

Findings:

  • Four percent (5 out of 120) of patients with ischemic thalamic stroke experienced isolated time disorientation.
  • Patients described this as 'time blindness,' indicating a profound inability to estimate or guess the correct time.

Implications:

  • Acute thalamic chronotaraxis is a distinct clinical syndrome associated with small artery disease affecting the mediodorsal nucleus of the thalamus.
  • This clinical presentation appears to have a favorable prognosis for recovery.
