Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...

Summary

Transvaginal natural orifice translumenal endoscopic surgery (NOTES) nephrectomy was successfully performed in a porcine model. This novel approach utilized a single port for kidney removal, demonstrating feasibility for future urologic applications.

Area of Science:

  • Urologic Surgery
  • Minimally Invasive Surgery
  • Natural Orifice Translumenal Endoscopic Surgery (NOTES)

Background:

  • Natural orifice translumenal endoscopic surgery (NOTES) has not been previously applied to urologic procedures.
  • This study introduces the first experience with transvaginal NOTES nephrectomy.

Purpose of the Study:

  • To evaluate the feasibility of a transvaginal single-port NOTES nephrectomy.
  • To demonstrate the application of purpose-built equipment in urologic NOTES.

Main Methods:

  • An acute experiment was conducted in a female farm pig.
  • A single 12-mm trocar was used for transvaginal access, accommodating the TransPort Multi-Lumen Operating Platform.
  • The procedure involved endoscopic dissection, vascularTransaction, and intact kidney retrieval via the vagina.

Main Results:

  • The transvaginal NOTES nephrectomy was successfully completed in the porcine model.
  • The total operative time was 300 minutes.
  • The kidney was retrieved intact through the vaginal route.

Conclusions:

  • Transvaginal NOTES nephrectomy is achievable in a porcine model.
  • Further studies involving survival animals are required to validate this surgical approach.
