Summary

Second lung re-transplants (re-LTx) are increasing. This study shows good outcomes using extended criteria donor lungs from donation-after-cardiac-death donors for re-LTx.

Background:

  • Bronchiolitis obliterans syndrome (BOS) is a leading cause of lung re-transplant (re-LTx).
  • Allocation of scarce donor lungs for re-LTx, particularly second re-LTx, presents a significant challenge.
  • The utility of extended donor criteria, including donation-after-cardiac-death (DCD) donors, for re-LTx is largely unknown.

Observation:

  • This report details a successful second lung re-LTx.
  • The donor lungs utilized were from a Maastricht category IV donation-after-cardiac-death donor with significantly extended criteria.
  • The patient experienced good clinical outcomes post-transplantation.

Findings:

  • Extended criteria donor lungs, including those from DCD donors, can be successfully used for second lung re-LTx.
  • This approach offers a viable option for managing the increasing demand for lung re-LTx.
  • Good clinical outcomes were achieved despite the use of significantly extended donor criteria.
