Observation:

  • An 18-year-old male presented with testicular pain and a mass in his atrophic right testis.
  • Ultrasonography identified a heterogeneous testicular mass.
  • Standard tumor markers and chest imaging were unremarkable.

Findings:

  • Pathologic examination revealed a teratoma with a nephroblastoma (Wilms' tumor) component.
  • Focal embryonal carcinoma and yolk sac tumor components were also identified.
  • This represents an extremely rare instance of Wilms' tumor arising within a testicular neoplasm.

Implications:

  • The co-occurrence suggests a shared or related molecular pathogenesis, potentially involving WT1 dysregulation.
  • This case expands the known spectrum of testicular tumors and their developmental origins.
  • Further research into WT1 gene activity in testicular embryogenesis may elucidate rare tumor formations.
