Summary

A challenging common bile duct stone required a two-step approach. Laparoscopic common bile duct exploration successfully removed the stone after initial endoscopic retrograde cholangiopancreatography failed.

Background:

  • Common bile duct stones (choledocholithiasis) are a frequent clinical problem.
  • Endoscopic retrograde cholangiopancreatography (ERCP) is a primary modality for stone removal.

Observation:

  • A 43-year-old male patient presented with a difficult common bile duct stone.
  • Initial ERCP allowed only stent placement, failing to clear the stone.

Findings:

  • The patient subsequently underwent successful laparoscopic cholecystectomy.
  • Laparoscopic common bile duct exploration (LCBDE) with primary closure after choledochotomy achieved stone clearance.

Implications:

  • LCBDE is a viable and effective alternative for managing complex common bile duct stones.
  • This case highlights the importance of considering alternative surgical approaches when endoscopic interventions are unsuccessful.
