Summary

This study compared two diffractive multifocal intraocular lenses (MIOLs), finding both effective for distance and near vision. Tecnis MIOLs reduced aberrations, while ReSTOR MIOLs showed better capsular biocompatibility for long-term stability.

Area of Science:

  • Ophthalmology
  • Biomedical Engineering
  • Optics

Background:

  • Cataract surgery often involves intraocular lens (IOL) implantation to restore vision.
  • Multifocal intraocular lenses (MIOLs) aim to provide functional vision at multiple distances.
  • Evaluating the performance and biocompatibility of different MIOL designs is crucial for patient outcomes.

Purpose of the Study:

  • To compare the functional vision (distance and near), optical quality (wavefront error), and biocompatibility of two diffractive multifocal intraocular lenses (MIOLs).
  • Specifically, to evaluate the Tecnis ZM900 MIOL against the AcrySof ReSTOR MIOL in patients undergoing cataract surgery.

Main Methods:

  • A prospective study involving 28 eyes of 28 cataract patients.
  • Phacoemulsification and implantation of either Tecnis ZM900 MIOL (Group 1) or AcrySof ReSTOR MIOL (Group 2).
  • Evaluation over 6 months included visual acuity (high/low contrast, distance/near), defocus curve, wavefront error, and capsule opacification.

Main Results:

  • No significant differences in distance visual acuity (high/low contrast) between the two MIOL groups.
  • Both MIOLs provided a depth of focus of 4.5 diopters.
  • Tecnis MIOLs (Group 1) demonstrated significantly lower total, spherical, and coma aberrations compared to ReSTOR MIOLs (Group 2).
  • A higher incidence of anterior capsule fibrosis was noted with Tecnis MIOLs, while posterior opacification was minimal and similar in both groups.

Conclusions:

  • Both diffractive MIOLs effectively improved distance and near functional vision.
  • Tecnis MIOLs offered superior optical quality due to reduced spherical aberration.
  • ReSTOR MIOLs exhibited higher capsular biocompatibility, potentially ensuring greater long-term stability.
