Summary

Major depressive disorder (MDD) patients with pain complaints are younger, more diverse, and report more anxiety and GI issues. Pain in MDD does not worsen depression chronicity or psychiatric burden.

Background:

  • Pain is a common symptom in Major Depressive Disorder (MDD).
  • Limited research exists on how MDD patients with pain complaints differ from those without.
  • Understanding these differences is crucial for targeted treatment.

Purpose of the Study:

  • To compare sociodemographic, clinical, and symptom features of MDD outpatients with and without pain complaints.
  • To identify specific characteristics associated with pain in the MDD population.

Main Methods:

  • Utilized baseline data from 3745 outpatients with nonpsychotic MDD in the STAR*D study.
  • Assessed pain complaints using Item 25 (somatic pain) of the Inventory of Depressive Symptomatology-Clinician Rating (IDS-C(30)).
  • Adjusted for covariates including sex, depression severity, and medical comorbidities.

Main Results:

  • Patients with pain complaints were younger, more likely to be African American or Hispanic, and less educated.
  • Pain was associated with anxious features, irritable mood, gastrointestinal problems, and poorer quality of life.
  • Pain complaints were not linked to a more chronic illness course or suicidal ideation.

Conclusions:

  • Pain is prevalent in MDD outpatients and linked to specific symptoms and reduced quality of life.
  • Despite associated features, pain in MDD does not appear to increase psychiatric burden or depression chronicity.
  • These findings highlight the need to address pain in MDD management for improved patient outcomes.
