Combining Reflectance Confocal Microscopy with Optical Coherence Tomography for Noninvasive Diagnosis of Skin Cancers via Image Acquisition

Background:

  • Filiform hyperkeratosis (FH) is a rare skin condition characterized by keratotic spicules.
  • Its association with neoplasms is debated, and it's not a well-defined paraneoplastic disorder.

Observation:

  • A 72-year-old patient presented with a three-month history of bilateral palmar lesions.
  • No other body surface lesions or abnormal findings from complementary exams were noted.

Findings:

  • Histopathological examination revealed parakeratotic columns, a reduced granular layer, and mild dermal inflammation.
  • The case presented exclusive palmar involvement without underlying malignancy.

Implications:

  • FH may be distinct from porokeratoses due to unique clinical and histological features.
  • Despite unclear links to neoplasms, thorough investigation for underlying conditions is recommended for FH cases.
