Background:

Atrioventricular (AV) canal defects are common congenital heart abnormalities.
Current surgical repair often involves prosthetic patches.
A patchless technique could offer advantages in specific patient populations.

Purpose of the Study:

To describe a new surgical technique for complete AV canal repair without a patch.
To evaluate the initial safety and efficacy of this novel method.

Main Methods:

Direct closure of the ventricular septal defect (VSD) component by suturing bridging leaflets to the ventricular septum crest.
Closure of the ostium primum defect using a running suture to the newly formed AV valve annulus.
Surgical repair performed on three pediatric patients.

Main Results:

No mortality observed in the three patients operated upon.
Mean ischemic time was 39 minutes and mean pump time was 77 minutes.
All patients maintained sinus rhythm post-operatively with only trivial or mild mitral regurgitation at follow-up.

Conclusions:

This new technique allows for complete AV canal repair without the need for a patch.
The described method is fast, simple, and reproducible.
Initial results suggest this patchless approach is a safe and effective alternative for AV canal repair.
