Background:

Plasma cell leukemia (PCL) is a rare and aggressive hematologic malignancy.
Understanding PCL's clinical presentation and treatment response is crucial for improving patient outcomes.

Observation:

This study analyzed four PCL cases among 152 multiple myeloma patients diagnosed between 1969 and 1988.
Three PCL cases were primary, and one was secondary.
Observed lower incidences of lymphadenopathy and splenomegaly compared to previous reports.

Findings:

The incidence of serum M band in PCL was comparable to multiple myeloma.
Only one of four patients achieved a partial response (PR) lasting two months with combination chemotherapy.
The mean survival duration for these PCL patients was under eight months.
