Physical Assessment of the Respiratory Tract I: Health History01:28

Physical Assessment of the Respiratory Tract I: Health History

Physical assessment of the respiratory tract is critical to patient care. It allows healthcare professionals to identify and manage various respiratory conditions. The process involves a combination of subjective and objective data collection.
Subjective Data
Subjective data provides vital information about the patient's health history and symptoms. This data is typically collected through interviews in which patients describe their experiences, symptoms, and concerns.
Health history and key...

Anatomy of Respiratory System I: Upper Respiratory Tract

The upper respiratory tract plays a vital role in the respiratory system, comprising several structures that facilitate air intake and prepare air for the lungs. It also serves as the first line of defense against pathogens and particles. This tract includes the nose and nasal cavity, the oral cavity, the paranasal sinuses, and the pharynx, each with specific functions and features.
Nose and nasal cavity
The nose and nasal cavity represent the main external openings of the respiratory tract.

Respiratory System Abnormal Finding I: Inspection and Percussion

Respiratory system abnormalities are a significant concern in healthcare due to their potential to indicate underlying severe conditions like Chronic Obstructive Pulmonary Disease (COPD), asthma, and pneumonia. These abnormalities can often be detected through physical examination methods like inspection and percussion.
Inspection Findings
During an inspection, several findings may suggest the presence of respiratory distress or disease. Pursed-lip breathing, where exhalation is slowed by...
Tonsillitis I: Introduction01:30

Tonsillitis I: Introduction

Tonsillitis is inflammation of the tonsils, which are two lymphoid tissue masses at the back of the throat. This condition can cause discomfort and irritation in the throat.
Etiology
Three primary contributing factors have been identified.
Acute Pharyngitis01:30

Acute Pharyngitis

Introduction
Acute pharyngitis is the inflammation of the back of the throat (pharynx), commonly resulting in a sore throat. It is a frequently encountered condition that prompts individuals to seek medical advice.
Classification
Acute pharyngitis can be categorized based on its underlying cause:
Chronic Pharyngitis01:23

Chronic Pharyngitis

Chronic pharyngitis refers to persistent inflammation of the pharyngial mucosa.
Etiology
It often arises from persistent viral or bacterial infections affecting sinuses and tonsils.
Additional contributing factors include inadequate dental hygiene, mouth breathing, recurring tonsillitis, allergic rhinitis, laryngopharyngeal reflux, and exposure to smoke, chemicals, and other environmental pollutants. Allergic reactions to pollen, mold, and pet dander, chronic cough, excessive voice usage,...

Summary

Chronic rhinosinusitis symptoms show poor correlation with objective sinus examinations. Both subjective symptoms and objective findings are crucial for assessing severity and guiding treatment in CRS patients.

Area of Science:

  • Otolaryngology
  • Rhinosinusitis Research
  • Medical Imaging in ENT

Background:

  • Chronic rhinosinusitis (CRS) is a common condition affecting quality of life.
  • Objective measures like CT scans and endoscopy are used to assess CRS severity.
  • The correlation between patient-reported symptoms and objective findings in CRS remains an area of investigation.

Purpose of the Study:

  • To investigate the relationship between patient-reported symptoms of chronic rhinosinusitis and objective findings from sinus examinations.
  • To determine if symptom severity correlates with objective measures of disease extent.

Main Methods:

  • Prospective data from 75 CRS patients scheduled for surgery were retrospectively analyzed.
  • Preoperative computed tomography (CT) scans were graded using the Lund-Mackay scale.
  • Nasal endoscopy was performed and graded using the Lanza and Kennedy score.
  • Patient symptoms were assessed using a visual analog scale (VAS).

Main Results:

  • A strong correlation was observed between CT scan and endoscopy scores (r = 0.88, P < 0.01).
  • Olfactory dysfunction severity correlated with disease severity on CT and endoscopy (r = 0.57, 0.53; P < 0.01).
  • Other CRS symptoms and overall symptom severity showed no significant correlation with objective measures (P > 0.05).
  • A weak correlation was found between VAS scores and CT findings (r = 0.26, P < 0.05), but not with endoscopy scores (P > 0.05).

Conclusions:

  • Patient-reported symptoms of chronic rhinosinusitis do not strongly correlate with objective examination findings.
  • Objective assessments (CT, endoscopy) and subjective symptom reporting are both essential for comprehensive CRS evaluation.
  • A combined approach considering both symptoms and objective data is necessary for appropriate treatment selection in CRS.
