Gallbladder

The gallbladder is a small, pear-shaped organ that plays a crucial role in our digestive system. Measuring about 10 cm in length, it is comparable in size to a kiwi fruit and is located in a hollow area on the lower surface of the liver. The gallbladder's primary function is to store and concentrate bile, a fluid produced by the liver that aids in digestion.
The gallbladder's anatomy consists of three regions: the fundus, body, and neck. Extending from the neck, the cystic duct joins the common...

Summary

Gall bladder cancer management in Nepal shows limited treatment efficacy. Radical surgery offers the only long-term survival for resectable gallbladder carcinoma, highlighting the need for randomized trials.

Area of Science:

  • Gastroenterology
  • Surgical Oncology
  • Oncology

Background:

  • Gallbladder cancer is a common biliary tract malignancy with significant geographical variations.
  • Its incidence appears relatively high in Nepal, necessitating local outcome reviews.
  • Biliary tract symptoms and chronic cholelithiasis are common presenting complaints.

Purpose of the Study:

  • To review the management outcomes and staging of gallbladder cancer.
  • To evaluate treatment efficacy in a Nepalese specialist cancer center setting.
  • To identify factors influencing survival in gallbladder carcinoma patients.

Main Methods:

  • Retrospective analysis of 68 histologically confirmed gallbladder cancer cases.
  • Data collected from January 1999 to December 2003.

  • Review of patient presentation, staging, treatment modalities, and outcomes.
  • Main Results:

    • 57% presented with biliary tract symptoms; 49% had a history of chronic cholelithiasis.
    • Only 21 patients had apparently resectable disease, with radical surgery demonstrating long-term survival.
    • Adjuvant therapies showed no impact; palliative chemotherapy had some response. Current treatments have limited efficacy.

    Conclusions:

    • Radical surgery in operable cases is crucial for long-term survival in gallbladder cancer.
    • Current treatment options, aside from radical surgery, have minimal impact on disease outcomes.
    • There is an urgent need for randomized data and further clinical trials for gallbladder carcinoma.
