Background:

Malaysia's drug control strategy has historically been punitive, aiming for a drug-free society by 2015.
Despite this approach, the nation has experienced a significant increase in illicit drug use and associated health crises.

Purpose of the Study:

To assess the drug use situation in Malaysia through a desk-based review.
To examine recent policy developments and their origins regarding drug use and harm reduction.

Main Methods:

A 3-week desk-based situation assessment was conducted in 2004.
Additional policy developments from 2005 were analyzed to understand recent changes.

Main Results:

A substantial rise in drug users and a high prevalence of HIV/AIDS among injecting drug users (IDUs), accounting for over two-thirds of cases.
Widespread high-risk drug use practices were identified, alongside the recent introduction of harm reduction initiatives.

Conclusions:

The successful piloting of substitution therapies, such as methadone and buprenorphine, indicates potential for effective harm reduction.
Government announcements in 2005 to expand methadone maintenance and establish needle/syringe exchange programs signal a shift towards evidence-based interventions for IDUs.
