Background:

  • The Accreditation Council for Graduate Medical Education (ACGME) established 6 core competencies for resident education in 1999.
  • The operating room (OR) is a traditional site for surgical resident teaching.
  • This study evaluated the OR as a setting for teaching and assessing these competencies.

Purpose of the Study:

  • To determine if the OR is an appropriate setting for teaching and assessing the ACGME competencies.
  • To quantify the frequency of teaching and demonstration of each competency within the OR.

Main Methods:

  • An observational study involving a 3-person team.
  • 11 surgical operations were observed to record teaching events.
  • Teaching events were categorized by faculty instruction or resident demonstration of specific competencies.

Main Results:

  • Patient Care was the most frequently taught (33% of operative time) and demonstrated (65% of the time) competency.
  • Interpersonal/Communication Skills and Practice-Based Learning/Improvement were occasionally demonstrated (4% each).
  • Other competencies were addressed less frequently in the OR setting.

Conclusions:

  • The OR is effective for teaching and assessing the Patient Care competency.
  • Current OR teaching practices are insufficient for addressing all 6 ACGME competencies.
  • Alternative educational approaches or settings, like clinics, are needed for other competencies.
