Area of Science:

  • Endocrinology
  • Surgical Oncology
  • Thoracic Surgery

Context:

  • Retrosternal goiters represent a significant subset of surgical thyroid disease.
  • Understanding their demographics and clinical presentation is crucial for surgical planning.

Purpose:

  • To analyze the characteristics, diagnosis, and surgical outcomes of patients with retrosternal goiters.

Summary:

  • This study reviewed 363 goiter surgeries, identifying 60 retrosternal goiters (16.5%). Predominantly affecting females (2.5:1) and individuals over 40 (68%), these goiters were often multinodular and long-standing.
  • Common locations included bilateral (37%) and left-sided (43%). Malignancy was found in 7% and thyrotoxicosis in 8%. Most patients (97%) were symptomatic.
  • Diagnosis was straightforward via physical exam, chest X-ray, and scintigraphy, with CT aiding complex cases. Surgery is indicated unless high-risk, with the cervical approach used in 98% of cases, yielding excellent safety outcomes.

Impact:

  • Highlights the prevalence and typical patient profile for retrosternal goiters.
  • Confirms the safety and efficacy of the cervical surgical approach for retrosternal goiters.
  • Provides data on associated risks like cancer and thyrotoxicosis, aiding preoperative assessment.
