Area of Science:

  • Orthopedic Surgery
  • Hip Reconstruction
  • Arthroplasty

Background:

  • McMurray's osteotomy is a procedure sometimes followed by total hip arthroplasty (THA).
  • The outcomes of secondary THA after McMurray's osteotomy require further investigation.
  • Understanding challenges in these cases can optimize patient care.

Purpose of the Study:

  • To evaluate the outcomes of total hip arthroplasty (THA) performed as a secondary procedure following McMurray's osteotomy.
  • To identify factors influencing the success and challenges of this surgical sequence.

Main Methods:

  • Retrospective analysis of 15 cases undergoing THA after McMurray's osteotomy.
  • Assessment of pain, function, hip stiffness, operative difficulty, and complications.
  • Evaluation of femoral component selection and soft-tissue management strategies.

Main Results:

  • 14 out of 15 patients experienced significant pain relief and functional improvement.
  • Hip stiffness was more pronounced compared to primary THA.
  • Delayed THA increased operative difficulties and reduced overall improvement; medullary canal distortion occurred in 8 cases without fractures.
  • Extensive soft-tissue dissection aimed to restore biomechanics and minimize loosening.

Conclusions:

  • Secondary THA after McMurray's osteotomy can yield good pain and functional results.
  • Delayed intervention and medullary canal distortion present challenges.
  • Trochanteric osteotomy, antibiotic prophylaxis, and preoperative planning are recommended for severely distorted canals.
