Erythrocyte swelling, measured by mean corpuscular volume (MCV), occurred during haemodialysis (HD) initiation, ultrafiltration, and hypotension. This erythrocyte edema may signal bioincompatibility or blood volume changes during dialysis.

Area of Science:

  • Nephrology
  • Hematology
  • Physiology

Background:

  • Erythrocytes are the most abundant blood cells and can be affected by haemodialysis (HD).
  • Previous understanding of intradialytic erythrocyte changes, specifically mean corpuscular volume (MCV), requires further investigation.

Purpose of the Study:

  • To re-examine intradialytic changes in erythrocyte water content, assessed by MCV.
  • To determine if HD, ultrafiltration (UF), or hypotension influences MCV.

Main Methods:

  • MCV was measured in 12 males and 8 females undergoing 81 HD sessions (47 uncomplicated, 34 with hypotension) and 16 isolated UF procedures.
  • Measurements were taken before and during dialysis, with analysis independent of intracellular and plasma sodium and potassium levels.

Related Experiment Videos

Main Results:

  • MCV did not significantly change after uncomplicated HD.
  • A significant increase in MCV was observed at the initiation of HD, during UF, and in cases of hypotension.
  • These MCV increases were independent of sodium and potassium concentrations.

Conclusions:

  • Erythrocyte edema, indicated by increased MCV, occurs during HD initiation, UF, and hypotension.
  • This phenomenon may be a response to bioincompatibility reactions or decreased blood volume.
  • Further research is needed to explore the potential hormonal mechanisms behind intradialytic MCV changes.
