    Summary

This study reveals four distinct types of systemic osteoporosis in women, differentiating bone loss mechanisms from age-related bone atrophy. Findings clarify bone remodeling disturbances and fracture risks, even with normal iliac bone density.

    Area of Science:

    • Bone biology
    • Osteoporosis research
    • Histomorphometry

    Background:

    • Systemic osteoporosis affects numerous women, leading to significant bone fragility.
    • Understanding distinct osteoporosis subtypes and their underlying mechanisms is crucial for effective treatment.

    Purpose of the Study:

    • To investigate and classify types of systemic osteoporosis in women using histomorphometric analysis.
    • To differentiate bone loss mechanisms in systemic osteoporosis versus age-related bone atrophy.
    • To correlate quantitative bone parameters with trabecular microanatomy to assess remodeling disturbances.

    Main Methods:

    • Histomorphometric analysis of iliac bone biopsies from 53 women with systemic osteoporosis.
    • Quantitative evaluation of osseous tissue parameters.
    • Comparison of microanatomical features of trabeculae with quantitative data.

    Main Results:

    • Identification of four distinct types of systemic osteoporosis.
    • Demonstration of differing bone tissue loss mechanisms between systemic osteoporosis and age atrophy.
    • Verification of disturbed resorption and osteogenesis in each osteoporosis type.
    • Evidence of potential fractures despite normal spongy bone density in iliac biopsies.

    Conclusions:

    • Systemic osteoporosis presents with diverse subtypes and mechanisms.
    • Iliac bone biopsy analysis provides critical insights into bone remodeling.
    • Fracture risk assessment requires consideration beyond standard iliac bone density measurements.
