Background:

Germ cell tumors (GCTs) are rare neoplasms that can occur in the brain.
Intracranial hypertension and cranial nerve palsies are serious neurological manifestations.

Observation:

A 24-year-old patient presented with chronic intracranial hypertension, abducens nerve palsy, impaired upward gaze, and right-sided hypoesthesia.
Imaging revealed a left thalamic tumor infiltrating the cerebral peduncle.

Findings:

Stereotactic biopsy confirmed the diagnosis of a germ cell tumor.
Following radiotherapy, the tumor completely resolved on CT and MRI scans.
The patient showed significant clinical recovery, with residual ophthalmoplegia.

Implications:

This case highlights the efficacy of radiotherapy in treating intracranial germ cell tumors.
Early diagnosis and prompt treatment are crucial for managing these rare brain tumors.
Understanding the long-term neurological sequelae, such as ophthalmoplegia, is important for patient management.
