Summary

This study found that 1.4% of individuals over 45 had chronic open-angle glaucoma. Advanced glaucoma significantly increases the risk of severe visual field defects.

Area of Science:

  • Ophthalmology
  • Public Health

Background:

  • Chronic open-angle glaucoma (COAG) is a leading cause of irreversible blindness worldwide.
  • Early detection and management are crucial to prevent vision loss.

Purpose of the Study:

  • To determine the prevalence of COAG with visual field defects (VFD) in a Swedish municipality.
  • To identify risk factors associated with advanced visual field defects (AVFD).

Main Methods:

  • A case-finding study was conducted in a central Swedish municipality.
  • Prevalence was calculated for individuals aged 45 and older.
  • Factors such as age, disease duration, intraocular pressure (IOP), and VFD stage were analyzed.

Main Results:

  • 128 cases of COAG with VFD were identified, with a prevalence of 1.4% in those >= 45 years.

Related Experiment Videos

  • 63% of cases had capsular glaucoma.
  • 49 patients had AVFD (visual field restricted to <= 20 degrees diameter); 4 had bilateral AVFD.
  • Patients with AVFD were older, had longer disease duration, higher initial IOP, and more extensive VFD at diagnosis.
  • VFD stage III at diagnosis increased AVFD risk 14-fold (adjusted for age/duration).
  • IOP >= 35 mmHg increased the risk of VFD stage III by 8.6 times (adjusted for age/glaucoma type).
  • Conclusions:

    • The prevalence of COAG with VFD in this population is significant.
    • Early diagnosis and aggressive management of elevated IOP are critical to prevent advanced vision loss in glaucoma patients.
