Area of Science:

  • Gastroenterology
  • Surgical Oncology
  • Pathology

Background:

  • Gastrointestinal stromal tumors (GIST) are the most common mesenchymal neoplasms of the GI tract.
  • GISTs typically express the c-kit proto-oncogene protein (CD-117).
  • These tumors often present with symptoms like abdominal pain and anemia.

Observation:

  • A 58-year-old male presented with upper abdominal pain and anemia.
  • Preoperative imaging (endoscopy, barium meal) identified a submucosal tumor in the gastric fornix.
  • Endoscopic biopsy confirmed spindle-shaped cells characteristic of GIST.

Findings:

  • Surgical resection of the gastric fornix with Hiss angle reconstruction was performed.
  • Histologic examination confirmed GIST with 2-5 mitosis/50HPF.

  • Immunohistochemistry revealed c-kit, CD-34 positivity, and a Ki-67 labeling index of 30%.
  • Implications:

    • Surgical resection is a safe and effective treatment for GIST.
    • Accurate diagnosis relies on histopathology and immunohistochemistry to differentiate GIST from other mesenchymal tumors.
    • Chemotherapy with tyrosine kinase inhibitors (e.g., imatinib mesylate) is a recommended adjuvant therapy.
