Summary

Intrusive luxation, a severe dental trauma, often leads to pulp necrosis and root resorption in teeth with open apices. Early endodontic intervention and a multidisciplinary approach are crucial for managing such cases and preventing complications.

Area of Science:

  • Dentistry
  • Endodontics
  • Oral Surgery

Background:

  • Intrusive luxation is a severe dental trauma with a 100% rate of pulp necrosis in teeth with open apices.
  • High risk of inflammatory and replacement root resorptions necessitates prompt endodontic intervention to prevent or delay these complications.

Observation:

  • A 15-year-old patient presented with a completely intruded maxillary central incisor, with the incisal edge at the crown-root interface.
  • Surgical exposure of the crown revealed negative pulp sensitivity, indicating non-vitality.

Findings:

  • A multidisciplinary approach involving surgery, orthodontics, and endodontics was employed for crown exposure, orthodontic extrusion, and root canal therapy.
  • The root canal was definitively obturated 3 years and 6 months after the initial treatment, following successive changes of calcium hydroxide dressing.

Related Experiment Videos

Implications:

  • This case highlights the challenges of managing severe intrusive luxation, emphasizing the need for a coordinated treatment strategy.
  • The study questions the optimal duration for calcium hydroxide dressing in preventing and managing root resorption following intrusive luxation.
