Summary

Pleomorphic carcinoma (PC) producing hCG in breast cancer patients indicates a higher risk of metastasis. Early-stage PC with hCG expression shows aggressive behavior, leading to poor patient outcomes.

Background:

  • A case study of an 80-year-old woman with a history of breast cancer.
  • The patient presented with an abnormal CT shadow during follow-up.

Observation:

  • A rapidly growing mass was surgically removed, revealing a 4.8 x 4.0 cm tumor.
  • Histology showed a pleomorphic carcinoma (PC) with spindle and polygonal cells.
  • Immunohistochemistry confirmed human chorionic gonadotropin (hCG) expression in approximately 20% of tumor cells.

Findings:

  • The PC was diagnosed based on its histological features and hCG expression.
  • Despite early-stage diagnosis (1B) and chemotherapy, the patient developed liver and adrenal metastases within 6 months.
  • The patient succumbed to metastatic disease 1 year post-operation.

Implications:

  • Pleomorphic carcinoma with hCG expression is associated with an elevated risk of local recurrence and distant metastasis.
  • Immunohistochemical detection of hCG in PC may serve as a prognostic marker for aggressive disease.
  • This case highlights the aggressive nature of hCG-producing PC and the need for vigilant monitoring and potentially novel therapeutic strategies.
