Background:

Migraine is a common neurological disorder.
Posterior circulation ischemia involves the brainstem and cerebellum.
Basilar artery (BA) migraine is a subtype of migraine with brainstem aura symptoms.

Observation:

A study examined 9 patients with migraine and posterior circulation ischemia.
Inclusion criteria included brainstem/cerebellar infarcts/TIAs, vertebrobasilar angiograms, and migraine.
Patients with isolated occipital lobe ischemia or other vascular diseases were excluded.

Findings:

Seven of the 9 patients experienced strokes (infarcts) in the brainstem or cerebellum.
Angiography revealed basilar artery narrowing or occlusion in 5 patients; occlusion was temporary in 3.
Migraine onset occurred before ischemia in 7 patients and after ischemia in 2.

Implications:

"Basilar migraine" is not always benign and can lead to significant neurological deficits.
The presentation of headaches, transient ischemic attacks, and strokes varies.
Basilar artery occlusion can be a feature of basilar migraine and may be reversible.
