Summary

Advanced breast cancer with extensive lymph node involvement (pN3 [10] disease) shows distinct clinical features. Prognosis for these patients may be improving with current treatments.

Tumor Progression

Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.

  • pN3 (10) disease correlated with larger primary tumors (T3-4), lobular histology, and inflammatory breast cancer compared to pN1 disease.
  • No association was found between pN3 (10) disease and early-onset or estrogen receptor-negative breast cancer.
  • Five- and 10-year overall survival rates for pN3 (10) patients were 64% and 21%, respectively.
  • Conclusions:

    • The prognosis for breast cancer patients with pN3 (10) disease may be improving with contemporary treatments.
    • Molecular mechanisms driving aggressive lymphotropism appear distinct from those in early-onset or ER-negative breast cancer.
