Purpose of the Study:

  • To identify predictors influencing GPs' decisions regarding the presence or absence of dementia.
  • To explore factors affecting GPs' diagnostic confidence in dementia assessments.

Main Methods:

  • An observational study involved 107 patients and 64 GPs using the Dutch national dementia guideline.
  • Logistic regression analyses examined predictors of dementia diagnosis and GP diagnostic confidence.
  • Data collected included clinical, patient-related, and GP-related variables, alongside guideline application.

Main Results:

  • Dementia was diagnosed in 67% of cases. Key predictors for dementia diagnosis included cognitive impairment, absence of depression, and patient's female gender.
  • GPs reported diagnostic confidence in 58% of cases. Confidence was associated with guideline adherence, functional dependency, longer assessment duration, informant availability, behavioral symptoms, and patient's female gender.
  • The use of the Mini Mental Status Examination did not correlate with diagnostic confidence.

Conclusions:

  • GPs appear to diagnose dementia based on logical clinical reasoning.
  • Adherence to dementia guideline recommendations may enhance GPs' diagnostic confidence.
