Recorded and reported sleep in chronic primary insomnia.

B L Frankel, R D Coursey, R Buchbinder

    Archives of General Psychiatry
    |May 1, 1976

    Summary

Chronic primary insomnia patients exhibit poorer sleep quality and longer sleep onset times compared to controls. Subjective sleep reports often misalign with objective polygraph findings in these individuals.

    Area of Science:

    • Sleep Medicine
    • Neuroscience
    • Psychiatry

    Background:

    • Chronic primary insomnia is a prevalent sleep disorder.
    • Objective and subjective sleep assessments can differ.

    Purpose of the Study:

    • To compare objective sleep measures and subjective sleep perceptions in chronic primary insomniacs versus controls.
    • To investigate discrepancies between subjective and objective sleep data.
    • To examine differences in sleep variability and insomnia subtypes.

    Main Methods:

    • Sleep polygraphy and questionnaires were administered to 18 chronic primary insomniacs and 18 matched controls.
    • Objective sleep parameters (latency, total sleep, efficiency, wake time, delta sleep) were recorded.
    • Subjective sleep quality and night-to-night variability were assessed.

    Main Results:

    • Insomniacs showed significantly longer sleep latencies, less total sleep, reduced sleep efficiency, more terminal wake time, and less delta sleep.
    • Significant discrepancies existed between subjective and objective sleep assessments in both groups, but in opposite directions.
    • Insomniacs' sleep exhibited greater night-to-night variability.
    • Controls reported worse sleep in the lab than at home, unlike insomniacs.
    • Differences between insomnia subtypes correlated with subjective complaints.

    Conclusions:

    • Objective sleep polygraphy reveals distinct physiological sleep disturbances in chronic primary insomnia.
    • Subjective-objective sleep discrepancies are a key feature, highlighting the complexity of insomnia perception.
    • Sleep variability and subtype differences offer insights into personalized insomnia management.
