Summary

Bilateral serous macular detachment can be an early sign of diffuse large B cell lymphoma (DLBCL). Vision improves with successful lymphoma treatment, indicating a favorable prognosis for these retinal lesions.

Background:

  • A 42-year-old male presented with subacute, bilateral, and progressive visual loss.
  • Medical history included chronic hepatitis C infection and Evans syndrome.

Observation:

  • Fundal examination revealed multiple areas of neurosensorial and retinal pigmented epithelium detachment in both maculae.
  • The patient later developed high fever and adenopathy, indicative of systemic illness.

Findings:

  • Biopsy of an enlarged lymph node confirmed diffuse large B cell lymphoma (DLBCL).
  • Following chemotherapy, the patient experienced significant bilateral visual improvement and reattachment of retinal lesions.

Implications:

  • Bilateral serous macular detachment can be an initial presenting sign of DLBCL.
