Autonomic function testing in children and adolescents with diabetes mellitus

Early detection of cardiac autonomic neuropathy (CAN) in type 1 diabetes mellitus (T1DM) is crucial. Autonomic dysfunction, indicated by abnormal heart rate variability (HRV) and baroreflex sensitivity (BRS), is detectable in young T1DM patients.

- Cardiac autonomic neuropathy (CAN) is a frequent complication of type 1 diabetes mellitus (T1DM), significantly increasing mortality risk.

Purpose of the Study:
- To investigate the presence and detectability of autonomic dysfunction in young individuals with recent-onset T1DM.
- To assess the utility of cardiorespiratory reflexes, heart rate variability (HRV), and baroreflex sensitivity (BRS) in diagnosing early CAN.

- Assessed autonomic function in 20 pediatric T1DM patients (10-19 years) and 136 controls using four cardiorespiratory reflexes.
- Utilized power spectral analysis of HRV (low- and high-frequency bands) and BRS via the Task Force Monitor.

Main Results:
- Cardiorespiratory reflexes were abnormal in 75% of T1DM patients and 60% of controls.
- A specific pattern of reduced BRS combined with abnormal HRV was observed in 30% of T1DM patients, but not in controls.
- No significant difference in HbA1c or diabetes duration was found between T1DM patients with impaired BRS and the overall diabetic group.
Conclusions:
- Autonomic dysfunction signs are prevalent even in the early stages of T1DM among young patients.
- HRV and BRS testing demonstrate higher specificity for diagnosing CAN compared to classical cardiorespiratory reflexes.
