Observation:

  • A postmenopausal woman with congenital aortic stenosis presented with severe iron deficiency anemia.
  • Extensive gastrointestinal analysis was negative for bleeding sources.
  • Octreotide treatment temporarily improved anemia but symptoms recurred upon cessation.

Findings:

  • A second-look capsule endoscopy and double balloon enteroscopy revealed an intestinal vascular malformation.
  • Surgical resection of the vascular malformation led to a sustained resolution of anemia.

Implications:

  • Intestinal vascular malformations should be considered in patients with unexplained iron deficiency anemia, even with congenital heart disease.
  • Multidisciplinary approaches involving cardiology, gastroenterology, and surgery are crucial for managing complex cases.
  • Successful surgical management of obscure gastrointestinal bleeding can significantly improve patient outcomes and quality of life.
