Summary

High cervical disc herniations at C2-C3 are rare. This study presents the anterolateral extradural approach as a viable surgical option for treating these challenging C2-C3 disc herniations.

  • A 72-year-old female presented with severe neck and occipital pain due to a left paramedian, extruded C2-C3 disc compressing the spinal cord and C3 nerve root.
  • Magnetic resonance imaging confirmed the C2-C3 disc herniation with significant neural element compression.

Findings:

  • A left-sided anterolateral extradural approach was successfully performed to resect the C2-C3 disc herniation.
  • The procedure involved mobilization of the V3 segment of the vertebral artery after resection of C1 and C2 transverse processes.
