Background:

  • Multiple Sclerosis (MS) patients often experience disabling tremor, with limited response to medical treatments.
  • Stereotactic surgery targeting the thalamus's vim nucleus is a known treatment for MS tremor.
  • Gamma knife radiosurgery (GK) offers a minimally invasive approach for tremor management, comparable to lesioning and deep brain stimulation (DBS).

Purpose of the Study:

  • To evaluate the efficacy and safety of Gamma Knife thalamotomy for managing disabling MS tremor.
  • To assess the functional outcomes and tremor reduction in patients undergoing GK thalamotomy.

Main Methods:

  • Six consecutive patients with disabling MS tremor underwent Gamma knife thalamotomy targeting the vim nucleus.
  • A single 4-mm isocenter delivered a median dose of 140 Gy to the thalamus.
  • Clinical outcomes were evaluated using the Fahn-Tolosa-Marin scale.

Main Results:

  • All patients showed tremor improvement after a median latency of 2.5 months, with a median follow-up of 27.5 months.
  • Tremor amplitude improved more significantly than fine motor skills like writing and drawing.
  • Four patients experienced functional improvement; one had transient hemiparesis, which resolved without lasting effects.

Conclusions:

  • Gamma knife radiosurgical thalamotomy is a safe and effective minimally invasive treatment for disabling MS tremor.
  • It serves as a viable alternative to conventional stereotactic surgery for palliative tremor management in MS.
