Summary
Cytologic features of primary pulmonary malignant peripheral nerve sheath tumor (MPNST) are described for the first time. Multinucleated giant cells and S-100 positivity aid in diagnosing this rare lung cancer.

Background:
- Malignant peripheral nerve sheath tumor (MPNST) is a rare soft tissue sarcoma.
- Histologic features of MPNST are known, but cytologic descriptions of primary pulmonary MPNST are lacking in literature.
Observation:
- A case of primary pulmonary MPNST in a 78-year-old male is presented.
- A 12 mm nodular lesion in the right upper lung lobe was identified.
- Frozen section revealed a malignant neoplasm, not otherwise specified.
Findings:
- Imprint cytology showed small cellular aggregates and discohesive neoplastic cells with malignant features.
- Histology revealed spindle cells with pleomorphic nuclei, multinucleated tumor giant cells, and >25 mitoses per 10 HPF.
- Tumor cells were positive for vimentin and S-100, with a Ki-67 labeling index of 10%.
Implications:
- MPNST exhibits varied cytomorphology with nuclear atypia and lack of definite differentiation.
- The presence of multinucleated neoplastic giant cells with S-100 immunopositivity can aid in the accurate diagnosis of primary pulmonary MPNST.
