Summary

Monopolar dissection tonsillectomy offers reduced operative time and blood loss compared to cold dissection. However, it may increase initial postoperative pain, with no significant difference noted later.

Background:

Tonsillectomy is a common pediatric surgical procedure.
Choosing the optimal tonsillectomy technique is crucial for patient outcomes.

Purpose of the Study:

To compare cold dissection and monopolar tonsillectomy.
Evaluate operative time, intraoperative bleeding, postoperative bleeding, and pain.

Main Methods:

100 children undergoing tonsillectomy were randomly assigned.
Both cold dissection and monopolar techniques were used on contralateral tonsils within the same patient.
Data on operative time, blood loss, and pain scores were collected.

Main Results:

Monopolar tonsillectomy showed significantly reduced operative time and intraoperative blood loss.
Average blood loss was 25.37 ml for monopolar vs. 88.5 ml for cold dissection.
Postoperative pain was significantly higher on the first day with monopolar technique (27% vs. 12%).

Conclusions:

Monopolar dissection tonsillectomy is a safe and effective alternative.
It offers advantages in reduced operative time and intraoperative hemorrhage.
While initial postoperative pain is higher, long-term pain is comparable to cold dissection.
