Culture-negative prosthetic joint infection (CN PJI) is uncommon and often follows prior antimicrobial therapy. Outcomes for CN PJI are comparable to those with known pathogens, especially with 2-stage exchange procedures.
Area of Science:
- Orthopedics
- Infectious Diseases
- Surgical Outcomes
Background:
- Culture-negative prosthetic joint infection (CN PJI) is a poorly understood complication.
- This study investigates the characteristics and outcomes of CN PJI.
Purpose of the Study:
- To define demographic characteristics of patients with CN PJI.
- To determine the treatment outcomes for CN PJI.
Main Methods:
- Retrospective cohort study of CN total hip and knee arthroplasty infections (1990-1999).
- Kaplan-Meier survival analysis was used to assess treatment success probability.
Main Results:
- 60 (7%) of 897 PJI episodes were identified as initial CN PJI.
- 53% of patients received prior antimicrobial therapy; 2-stage exchange was the most common treatment (57%).
- 5-year survival free of treatment failure was 94% for 2-stage exchange and 71% for debridement and retention.
Conclusions:
- CN PJI is infrequent but associated with favorable outcomes comparable to culture-positive PJI.
- Prior antimicrobial use is common in CN PJI cases.
- 2-stage exchange demonstrates high success rates for CN PJI treatment.
