  • Patients with Grade 3 PGD had significantly lower 5-year (51%) and 10-year (11%) survival rates compared to Grade 0-1 (66%, 38%) and Grade 2 (64%, 35%) PGD.
  • Bilateral lung recipients with Grade 3 PGD showed reduced BOS-free survival and worse mean FEV1 compared to those without severe PGD.
  • PGD grade did not correlate with post-transplant pulmonary function in single-lung recipients.

Conclusions:

  • Grade 3 PGD negatively impacts long-term survival in lung transplant recipients.
  • Severe PGD adversely affects BOS-free survival and pulmonary function in bilateral lung transplant recipients.
  • Early PGD is a critical factor influencing long-term outcomes in lung transplantation.
