Summary

A pyogenic granuloma, a rare complication of silicone punctal plugs used for dry eye syndrome, can develop due to chronic irritation. Removal of the plug resolves the condition.

Background:

Dry eye syndrome is a common condition often managed with punctal plugs.
Silicone punctal plugs are frequently used to retain tears by blocking the lacrimal punctums.

Observation:

A 65-year-old woman with dry eye syndrome presented with a red mass in the internal canthus of her left eye.
The mass was diagnosed as a pyogenic granuloma, occurring at the site of a previously inserted silicone punctal plug (Smart Plug).

Findings:

The pyogenic granuloma was initially removed, but recurred.
Subsequent removal of both the granuloma and the punctal plug resulted in complete resolution of symptoms.

Implications:

Pyogenic granuloma is a rare but potential complication associated with silicone punctal plugs.
