Summary

High-dose inhaled corticosteroids may effectively treat post-transplantation constrictive bronchiolitis (PTCB), a common complication after allogeneic hematopoietic stem cell transplantation (HSCT). This treatment stabilized lung function in patients with PTCB.

Background:

  • Post-transplantation constrictive bronchiolitis (PTCB) is a frequent pulmonary complication in allogeneic hematopoietic stem cell transplantation (HSCT) survivors.
  • PTCB is a late manifestation of graft-versus-host disease (GVHD) and its current treatment with systemic corticosteroids has significant side effects.
  • Topical corticosteroids are used for other GVHD manifestations to mitigate systemic side effects.

Purpose of the Study:

  • To evaluate the efficacy of high-dose inhaled corticosteroids for treating PTCB.
  • To assess the impact of inhaled corticosteroids on airflow obstruction in PTCB patients.

Main Methods:

  • Retrospective analysis of adult patients diagnosed with new-onset PTCB.
  • Patients received high-dose inhaled fluticasone propionate (500-940 mcg BID).
  • Forced expiratory volume in 1 second (FEV1) was monitored before and after treatment.

Main Results:

  • Seventeen patients with PTCB showed a significant decline in FEV1 post-HSCT (median 53%).
