Summary

This study identifies a safe threshold for remnant liver volume in hepatocellular carcinoma (HCC) patients undergoing hepatectomy to prevent post-operative liver failure. Achieving a higher ratio of remnant liver volume than the calculated value ensures safer liver resections.

  • A regression model established a relationship between indocyanine green retention rate (ICG15) and remnant liver volume (RR) in patients with liver dysfunction: RR = 1.98 x ICG + 0.3672 (r = 0.92, p < 0.001).

Conclusions:

  • Hepatectomies are safer when the estimated pre-operative ratio of remnant liver volume exceeds the calculated value from the derived equation.
  • This provides a quantitative guideline for surgeons to optimize hepatectomy extent and minimize liver failure risk.
