Summary

Mycosis fungoides (MF), a common skin lymphoma, has variable prognoses based on disease stage and spread. Early stages offer excellent survival, while advanced disease with extracutaneous involvement significantly reduces life expectancy.

Background:

  • Mycosis fungoides (MF) is the most common cutaneous T-cell lymphoma.
  • Sezary syndrome, though debated, is a distinct classification in the WHO-EORT lymphoma classification.
  • MF predominantly affects older adults, with a 2:1 male-to-female ratio.

Purpose of the Study:

  • To analyze the prognostic factors influencing survival in Mycosis fungoides.
  • To correlate disease stage, skin involvement, and extracutaneous spread with patient outcomes.

Main Methods:

  • Retrospective analysis of Mycosis fungoides patient data.
  • Correlation of clinical staging (IA, IB, IIA, etc.) with survival rates.
  • Assessment of extracutaneous dissemination and its impact on prognosis.

Main Results:

  • Stage IA MF patients have excellent long-term survival (>33 years), with most deaths unrelated to MF.
  • Patients with stage IB or IIA disease have a median survival over 11 years, with a 24% progression risk.
  • Advanced disease, including cutaneous tumors or erythroderma with extracutaneous spread (30-40%), significantly reduces median survival to 3-4.5 years or less than 1.5 years if lymph nodes/viscera are involved.

Conclusions:

  • Prognosis in Mycosis fungoides is strongly determined by disease extent and extracutaneous involvement.
  • Early-stage MF (Stage IA) has a favorable outlook, while advanced stages and visceral spread are associated with poor survival.
  • Development of CD30+ large cell transformation in plaque-type or erythrodermic MF indicates a less favorable outcome.
