Adjuvant interstitial brachytherapy (BTX) offers effective local control for soft tissue sarcomas (STSs). This treatment is well-tolerated in both low- and high-grade STS patients.

Area of Science:

  • Oncology
  • Surgical Oncology
  • Radiation Oncology

Background:

  • Soft tissue sarcomas (STSs) are rare, diverse cancers primarily treated with surgery.
  • Adjuvant radiation therapy, including brachytherapy (BTX), may be used based on tumor location and grade.

Purpose of the Study:

  • To evaluate treatment outcomes for soft tissue sarcomas (STSs) managed with surgery and adjuvant interstitial brachytherapy (BTX).
  • To assess the efficacy and toxicity of interstitial brachytherapy in STS management.

Main Methods:

  • Retrospective analysis of 43 patients with primary or recurrent STS treated with surgery and adjuvant BTX between 1997 and 2005.
  • Tumors included low-grade (12) and high-grade (31) STS, primarily larger than 5 cm.
  • Treatment protocols varied based on tumor grade and resectability, involving BTX alone, or combined with external beam radiation therapy (EBRT).

Main Results:

  • Median follow-up was 39 months, with 34 patients alive at last follow-up.
  • Overall local control rate was 88% (87% for high-grade, 92% for low-grade STS).
  • Disease-free survival was 75% and overall survival was 79%. Long-term toxicity >Grade 3 was 7%.

Conclusions:

  • Adjuvant interstitial brachytherapy (BTX) demonstrates acceptable local control for soft tissue sarcomas (STSs).
  • The treatment is generally well-tolerated in patients with both low- and high-grade STS.
  • Interstitial BTX is a viable option for managing STS, particularly when combined with surgery.
